5. (1) An application for registration of a data controller or data
processor shall—
(a) be in Form DPR1 set out in the First Schedule; and
(b) be accompanied by the registration fees specified in the
Second Schedule.
(2) An application for registration under sub-regulation (1) shall
be accompanied by—
(a) a copy of the establishment documents;
(b) particulars of the data controllers or data processors
including name and contact details;
(c) a description of the purpose for which personal data is
processed; and
(d) a description of categories of personal data being processed.
